Forms Gateway
Forms Gateway is an online data collection tool configured for any form used by your organization. Available for a variety of vertical markets – including education, city/state government and healthcare – Forms Gateway streamlines the time consuming, error prone data entry, enabling any business process to operate at maximum efficiencies.
To support electronic forms processing, the Gateway system enables the distribution, routing and on-line approval processes surrounding your web-based forms. Easily add supporting documents and retain as part of the permanent e-record. Processes requiring the acknowledgment and confirmation of policy and procedure are included in the Gateway system.
To further collaboration, the Gateway Scheduling Module streamlines scheduling of necessary meetings among stakeholders.

Dauphin County Technical School, Pa.
- Special Education updates from Teachers Module
- Testing Documentation Module – Handwritten test where students show their work so teachers can see a progression in learning. The school uses Forms Gateway to print out the forms which are encoded with a special bar code. Once the test is taken, it is scanned by the staff and automatically transferred into the students folder in the electronic repository. Teachers log into the system bi-weekly and will see a preloaded list of students assigned to them. Once the teacher completes the necessary information—an electronic version of the document will be routed for review by the case worker and archived.
The City of Newark, N.J.
- Trade Waste Module – The City of Newark uses the Gateway Forms module to operate their Trade Waste licensing program. The module has allowed the city to quickly collect information (such as what type of waste and who is their carrier for pickup) from local business owners who visit City Hall. Forms Gateway then instantly generates a pixel-perfect paper license with all of the data and a City of Newark watermark. This allows the business owner to immediately post the license at their business without having to wait for processing.
West Orange Public Schools, N.J.
- Education Technology Training Centers Module (ETTC) – The West Orange School District is a current Registration Gateway customer who is using the Gateway Platform to further leverage their investment. The district automated the registration/certification for the Education Technology classes offered to teachers to continue their education. Teachers from other districts who are interested in signing up for a technology training classes will login through Forms Gateway and register for a class. The registration is then processed and the teacher receives an invoice/PO. In addition, upon completion of the course the teacher will receive a certificate.
